Community Health Center (CHC) - Westshore

The Westshore Community Health Centre (WSCHC) is a new multidisciplinary primary care centre operated by Pacific Centre Family Services Association (PCFSA) which is designed to bring low-barrier, trauma-informed coordinated healthcare to the underserved populations of the Westshore and their families. With Family Physicians, Nurse Practitioners, Registered Nurses, and other allied health care providers, we provide comprehensive longitudinal health care to attached patients.
Priority populations for the WSCHC include individuals of all ages and their families who experience barriers to health/health care due to the impact of inequity and the Social Determinants of Health (SDOH) which include: income, employment and working conditions, education/literacy, childhood experiences, built environment, lack of social supports, gender/race/class based discrimination and the intersection of same.
New intakes to the Westshore CHC are temporarily paused and will likely resume in the fall. Thank you for your patience.

The catchment area for the Westshore Community Health Centre will include:
Colwood
Langford/Highland
Metchosin.
People who are within a priority population, who live in this catchment, and who do not currently have a primary care provider will be prioritized for attachment. People who live outside the catchment but within the Western Communities primary care network will be considered on a case-by-case basis depending on their service needs and the capacity of the Centre.
People who live outside the Western Communities PCN are NOTeligible for attachment to the WSCHC.
