Hospital at Home (HaH) - Interior Health

Provided by Interior Health

A program that enables hospital-level care to be provided in the comfort of a patient's home.
In the Hospital at Home program patients remain a hospital patient and the hospital ward is their own home. HaH aims to improve the patient, caregiver and health-care provider experience of care. It's an internationally recognized model of care, operating for over 20 years. Programs exist in other countries, and in other B.C. health authorities. Each day, patients will receive at least one in-person visit, along with regular virtual check-ins, ensuring consistent and personalized care. HaH offers:
  • 24/7 care through a combination of virtual and in-person visits to the home
  • Vital sign readings are taken in the home and communicated electronically to the care team
  • All required technology and medications are provided
  • There is no cost to patients on the program
  • Patients have a virtual call bell and phone number to connect with their care team 24/7

Patients must meet the following criteria to be considered for the HaH program:
  • Live within a 20-minute drive of the Kelowna General Hospital
  • Be admitted to KGH with a known medical diagnosis and treatment plan
  • Require medical care, but be in stable condition
  • Be 17 years of age or older (17- and 18-year-old patients require parental/guardian consent and presence while the patient is on the program)
  • Agree to participate in the program and receive in-person and virtual care
