Indigenous Health Care Social Workers that provided culturally-grounded support services, including care navigation and case management.
A dedicated team of Indigenous Health Care Social Workers committed to supporting the holistic wellness of Indigenous individuals, families, and communities. Together, they bring a shared passion for culturally grounded, trauma-informed, and strengths-based practice that honours the interconnected relationships between mind, body, spirit, family, and land. Services Provided:
  • Holistic psychosocial assessments rooted in Indigenous worldviews
  • Short-term counselling and emotional support
  • Health care case management and culturally informed system navigation
  • Advocacy for equitable, anti-racist, and culturally safe healthcare
  • Referrals to traditional healing, health and mental health services, community supports, and social programs
  • Assistance with housing, income supports, benefits, and practical resources
  • Wellness education grounded in Indigenous models of health and community resilience
  • Non-clinic support services that may include connection to Elders or cultural supports where appropriate. Supports are guided by the client’s goals and consent
This program is intended for Métis and Urban Indigenous individuals or families who may benefit from additional cultural, emotional or system navigation support alongside their existing care. The pathfinders work in a relationship-based practice, spending much of their time in the community and meeting with clients in a variety of settings, depending on the reason for the visit, the clients needs, confidentiality requirements, and safety for all involved. These locations can include the pathfinder's office, accompanying clients to other providers; offices, community-based resource provider locations, home visits, the hospital or in public spaces.

Referral: Referrals are for individuals or families who identify as Indigenous, including Métis, First Nations or Inuit, whether formally registered or self-identified. Referrals may be submitted by family physicians, nurse practitioners, nurses, allied health professionals or other service providers who are working with the client. Referrals should be made with the client’s knowledge and consent. Please note, that case load is full and referrals will result in clients being added to a waitlist.
