Indigenous Patient Navigator Services
Advocacy and Indigenous Services
Provided by Interior Health
Offers practical support to Indigenous persons utilizing Interior Health services through hospital liaison and referral processes. Serves all Aboriginal people, including First Nations status, First Nations non status, Inuit, and Métis.
Our purpose is to provide a communication linkage between the patient, family, community and health care provider, assisting the care team with health care, mental health, substance use and discharge planning.
Services provided for patients/clients: Help patients understand and access health services, provide connections to religious and spiritual services, provide advocacy and emotional support, and work as part of the multidisciplinary team to connect to services on discharge
Services are available through the hospital and/or the community. Service locations: Ashcroft, Castlegar, Cranbrook, Creston, Kamloops, Kelowna, Penticton, Vernon, Williams Lake.
Services are usually provided Monday - Friday during the day but can vary depending on the area of service. Services are provided in English and other Aboriginal languages.
No referral is required.
