Home Care Program

Provided by Carrier Sekani Family Services

Provides in home care and support to member First Nations based on individual medical needs. For on-reserve individuals dealing with chronic disease, end of life care, or returning home from a hospital stay.
Provides services predominantly to individuals who have recently been discharged from the hospital, or struggle with chronic disease. Services may include follow up care following a hospital visit, chronic illness monitoring, and health and nutrition education. Also provides support to individuals requiring health aids and equipment, and assess individuals for long-term care requirements if needed. Due to the nature of our services, the program mostly provides services to adults in the 30-59 and 60+ stages of the life cycle, but will also provide services to anyone in need at any age.

The team consists of a team of Residential Care Aids, Home Care Nurses, and Community Health Nurses that rotate throughout our member First Nations. Home Care services are currently provided on-reserve to: Wet’suwet’en, Burns Lake Band, Nadleh Whut’en, Cheslatta Carrier Nation, Nee Tahi Buhn, Yekooche First Nation, Skin Tyee, Stellat’en, Takla Lake First Nation and Saik’uz First Nation. The team collaborates to ensure culturally sensitive and holistic care is provided in each community.

  • Personal hygiene
  • Hospice/palliative care (end of life)
  • Diabetes management & diet advice
  • Blood glucose checks
  • Blood pressure checks
  • Massage
  • Mobility/exercise
  • Socialization
  • Terminal illness care
  • Respite
Services are provided free of charge to all Carrier and Sekani community members living on reserve who are eligible for Home Care services.

Home Care services are predominantly offered from Monday to Friday at various times; services for palliative care and respite service times vary based on client need.

Referrals are accepted from community members and professionals, and followed by an individualized assessment to determine specific needs.
