Provides specialized seniors health care for medically complex and physically frail older adults.
Seniors Health and Wellness Centre patients are referred by their family doctor, nurse practitioner, or specialist. The interprofessional team at the centre may include a geriatrician, family physicians with a special interest in geriatrics, mental health clinician, physiotherapist, occupational therapist, registered dietitian, rehabilitation assistant, speech and language pathologist, pharmacist, registered nurse, social worker, and respiratory therapist provide specialized short-term care (four to eight weeks).

Services offered at the Centre include:
  • Collaboration with Clients existing community support network (e.g. family physician / NP, Home and Community Care Case Manager) as well as linking client and family with services and resources (e.g. home and safety assessments; financial assessments; nutritional assessments; coordination and collaboration with home support)
  • Comprehensive Geriatric Assessment, customized inter-professional care planning with the client and the family, personalized interventions and short-term follow-up and transitioning to community services


    • Criteria: The program is intended for medically complex and physically frail older adults that would have:
  • 65 years of age or older (under 65 years by exception)
  • Clinical Frailty Scale score of 4-6
  • Medically complex (at risk of decline without interventions)
  • Aware and consents to Seniors Health and Wellness Centre referral, assessment and recommendations
  • Potential to stabilize

    Note:  For clients referred for confirmation of diagnosis of major neurocognitive disorder WITHOUT other health concerns, please consider referral to Seniors Mental Health and Substance Use
