Proof of an acquired brain injury confirmed by a physician and through medical or rehabilitation documentation is required. The following types of documentation are accepted as medical proof of acquired brain injury: CT scan, MRI report, neurologist's report, and physiatrist's report. Hospital records, medical rehabilitation reports, hospital discharge summaries, neuropsychological assessments, and psychiatric reports including a DSM-5 diagnosis of acquired brain injury are also accepted.
